RN Case Manager
Visiting Nurse & Hospice Home is seeking an RN Case Manager for home hospice care. This position provides patient care in the home or a nursing facility and participates in on-call rotation. This position requires coordination with the interdisciplinary team and the ability to recognize the needs and emergencies of hospice and palliative patients. Must have a current RN license to practice in Indiana, be CPR certified, have a valid Indiana driver’s license, current automobile insurance, excellent organizational skills and a commitment to superior customer service. EOE.
CNA – PRN / Hospice Home (in patient)
QUALIFICATIONS: Staffing needed for weekends and some weekdays in Hospice Home, the 14-bed in patient unit. Candidate must exhibit excellent communication skills with patients and families and must be able to work efficiently and complete documentation accurately in the Allscripts computer system. Must be a graduate of an accredited school of nursing and have a current CNA license to practice in the state of Indiana. Applicant must have ability to recognize needs and emergencies of hospice patients and provide appropriate interventions, be CPR certified, have a valid Indiana driver’s license, current automobile insurance and a commitment to superior customer service.
