Chaplain – Home Care – Full-time, 40 hours, Monday - Friday

QUALIFICATIONS: Chaplain needed to provide non-denominational spiritual counsel and emotional support to the active and bereaved hospice patients and their families; to serve as a liaison between community clergy and hospice; to provide support and spiritual counsel to members of the hospice team. Candidate must exhibit excellent communication skills and must be able to work efficiently and complete documentation accurately in the Allscripts computer system. A bachelor’s degree with experience in bereavement and grief and one (1) year of chaplaincy/counseling and/or previous experience in terminal care preferred (internship, clinical pastoral units or past work experience). Must have a Ministerial License and/or be Ordained. Candidate must be able to safely perform the essential functions of the job without posing a direct threat to the health and/or safety of self or others in the workplace, have a valid Indiana motor vehicle operator’s license, access to automobile and current automobile insurance coverage and must be able to drive independently to all patient/family clients’ and a commitment to superior customer service. CNA Home Care, Full –time; 12 noon – 8 pm, M-F 

QUALIFICATIONS: Staffing needed for hospice and palliative care in home and nursing facilities. Candidate must exhibit excellent communication skills with patients and families and must be able to work efficiently and complete documentation accurately in the Allscripts computer system. Applicant must have ability to recognize needs and emergencies of hospice patients and provide appropriate interventions, be CPR certified, possess a valid Indiana driver’s license and current automobile insurance and a commitment to superior customer service. High school diploma or GED required OR Completion of 76 hour Home Health Aide certification program (need copy of Certificate) OR Completion of 105 hour CNA Certification program (need copy of Certificate), with or without experience. Letter from State with Certificate number required AND Six (6) months (one [1] year preferred) CNA/HHA experience, within the last twenty-four (24) months and has been competency tested by another agency OR have six (6) months full time documented hospital experience within the last twenty-four (24) months OR RN nursing students who have successfully completed courses with a grade of “C” or better will be said to have met the above qualifications OR Less than six (6) months’ experience if the candidate has been certified by training programs approved by Visiting Nurse & Hospice Home.

 

CNA Home Care, PRN, Weekends

LPNs – PRN, Continuous Care

QUALIFICATIONS: LPNs needed for continuous care in home or nursing facility settings. Candidates must exhibit excellent communication skills with patients and families and must be able to work efficiently and complete documentation accurately in the Allscripts computer system. Must be a graduate of an accredited school of nursing and have a current LPN license to practice in the state of Indiana.  Applicant must have ability to recognize needs and emergencies of hospice patients and families and provide appropriate interventions, be CPR certified, possess a valid Indiana driver’s license and current automobile insurance and a commitment to superior customer service.
