Registered Nurse (RN) — evaluates and provides symptom management along with teaching the family skills to help the patient achieve comfort during the last phase of life.

Certified Nursing Assistant/Home Health Aide — assists with activities of daily living including personal care.

Volunteer/Companion — runs errands, provides companionship for patient, and offers needed restful intervals for caregiver.

Social Worker — assists in meeting patient/family needs when facing end-of-life issues, including identifying strengths, making referrals to community resources, and providing emotional support.

Spiritual Care Coordinator — provides spiritual counsel and support to hospice patients and their families.

Bereavement Coordinator — provides support for family and friends of hospice patients for one year following the death of their loved one, including referrals to support groups and community grief agencies.

Therapist — provides occupational, physical and speech therapy as needed to enhance quality of life.
