of daily living including personal care.
Volunteer/Companion
runs errands, provides companionship for patient, and offers
needed restful intervals for caregiver.
Social
Worker assists in meeting patient/family needs when facing
end-of-life issues, including identifying strengths, making referrals
to community resources, and providing emotional support.
Spiritual
Care Coordinator provides spiritual counsel and support
to hospice patients and their families.
Bereavement
Coordinator provides support for family and friends
of hospice patients for one year following the death of their loved
one, including referrals to support groups and community grief agencies.
Therapist
provides occupational, physical and speech therapy as needed
to enhance quality of life.
